I'll run the CCleaner reg cleaner if I think there's an issue. Like a program that did a messy uninstall. For registry optimize the only one I risk is NTREGOPT and even then, maybe once every couple of months just to smooth out the boot up.
Too much tinkering can get you a lot of fixing though.
edit: generally I go with, if it's working for you keep doin' it that way.
I try not to carry it too far though. Like Nomar Garciaparra at bat. That guy made me tired just watching him fix his gloves!!
