Windows XP help o.o

Ok, long story short, my little brother has Windows XP. His computer refuses to boot into Safe Mode of ANY kind (He selects it, tries to run with it, it resets itself) if you double click ANY folder on the Desktop and try to navigate to another one, it freezes the process (NOT THE PC) Avast won't scan, neither will his other programs. I just hooked his drive up to my PC, scanned it, Defragged it and only found 1 virus. Task manager doesn't work for him and the Ctrl+r combo doesn't work either.. Any ideas?
