I do have MS office home and student 2007, I am hoping to find a detailed tutorial somewhere on how to create the form along with the text. Thank you for your imput as it will help me find what I need.

Type your stuff in Word
Then click on the upper left hand corner. You will see save as. Click on that and then finally PDF
I am glad that you have Word.

Yes just made a normal txt file and used open office which gives the option to save as .pdf
