Baseline Security v. 2.2 where name etc?

Have Vista home premium 64 bit
Downloaded v. 2.2 Baseline Security
Completely different from earlier version
Now it says to enter name - But there is not a field to enter name.
OR enter ISP - tried that and the program still does not work.
How to go back to earlier version?
Or fix this one?

I have found a site that offers the old version

Unfortunately, I do not know the reputation of the site or how safe the download would be. Before you download, if they give you the option to save, save it to a DVD, then you can run an antivirus and malwarebytes (download this) before you proceed to download to your computer.
Not being familiar with the Software, I cannot tell you how to proceed with the newer version. You can conact support.
