Cannot add association for new extension

I'm trying to add a new extension. When I double click a file with that extension it can't find the program. I click to locate the program. It isn't in the standard list so I click browse, locate the program and click it. The program is not added to the standard list and no association is made.

Where is the data stored in the registry? Can I add it via regedit?

I'm trying to add .MYO to point to MYOB.EXE in the appropriate MYOB program folder.I do this to so that I can have an icon for each database.
